Orthopedics Policies and Forms
Insurance Information and Benefits: We accept most major employer-sponsored insurance plans. At this time, we do not participate with Medicare HMO plans or insurance plans obtained through federal or state exchange programs.
Please verify your insurance benefits before scheduling an appointment so you understand your coverage fully. As a courtesy, we verify insurance benefits related to surgery; however, we strongly encourage you to contact your insurance company directly. Ultimately, it is the patient’s responsibility to understand their insurance coverage and benefits.
We understand that insurance can be complicated and that obtaining clear answers may be difficult. Please do not hesitate to contact our office with questions related to your care at Orthopaedic Sports Specialists.
Appointments: We make every effort to remain on schedule and therefore cannot accommodate walk-in appointments. We appreciate your patience if we are running behind schedule.
Cancellation Policy: Our goal is to provide timely, high-quality medical care to all patients. If you need to cancel an office appointment, please notify us by 10:00 a.m. at least one business day in advance.
For non-medical surgical cancellations, we require at least one week’s notice for outpatient surgery and three weeks’ notice for total joint replacement surgery. A $50 fee may be charged for an office appointment cancelled without proper notice. A $500 fee may be charged for a surgery cancelled without the required notice.
Appointments are in high demand. Prompt cancellation allows us to offer the appointment time to another patient who may need timely care.
Second-Opinion Policy: To provide the best possible care for second-opinion consultations—or if you have been treated by another physician for the same condition—our clinical staff will review the relevant records before scheduling an appointment.
Please provide a cover sheet that includes your name, date of birth, address, telephone number, and a brief history of the injury or condition. This information helps us review your case efficiently and provide prompt feedback regarding your care.
Prescriptions & Renewals: Please request prescriptions and renewal authorizations between 8:30 a.m. and 5:00 p.m., Monday through Friday. Requests made outside these hours will be addressed only in urgent or emergent circumstances.
For routine renewals, call our main number and select option 3 for the prescription line. Please allow at least 48 hours for routine prescription renewals. If your request is urgent, do not leave a message on the prescription line, as it may not be monitored daily.
Forms: There is a $20 fee for each initial provider-completed form and a $10 fee for each update. Please complete the Acknowledgement of Fee Schedule for Provider-Completed Forms below so we can keep it on file for future form requests. Allow at least seven business days for form completion.
Fees & Payments: We are required to collect copayments at the time services are rendered. We accept cash, checks, and all major credit cards. If we participate with your insurance plan, we will submit claims directly to the insurer; however, payment remains the patient’s responsibility. We will assist whenever possible with claim-related questions.
Forms: The forms below may be required before your visit and/or surgery. To help expedite registration, please complete the applicable forms and bring them to your scheduled appointment. Thank you.
