Physical Therapy - Policies and Forms

Forms: The forms and policies below are provided for your convenience. To help expedite registration, please complete the applicable forms and bring them to your scheduled appointment. Thank you.

Insurance information: The Physical Therapy Department accepts most employer-sponsored insurance plans and exchange plans. At this time, we do not accept Medicaid plans.

If you have questions about your eligibility or benefits, please call the member-services number on the back of your insurance card. As a courtesy, we will verify your physical therapy benefits; however, we strongly encourage you to contact your insurance company directly. Ultimately, it is the patient’s responsibility to understand their insurance coverage and benefits.

Appointment Policy: Physical therapy can improve function and daily life through a variety of evidence-based treatments. Your physician and physical therapist will recommend an appropriate frequency of visits. Consistent attendance is important to achieving the best possible outcome. Frequent cancellations can adversely affect your progress.

We make every effort to schedule appointments in a manner that maximizes your time with the therapist. Arriving late may disrupt your visit, the therapist’s schedule, and the appointments of other patients.

We understand that circumstances may occasionally require an appointment to be rescheduled.

  • If you need to reschedule, we require at least 24 hours’ notice. Please call our office to arrange a make-up appointment with a Front Desk Receptionist. Whenever possible, the make-up appointment should occur during the same week, preferably the following day.

  • If you cancel or do not attend more than one appointment during a 12-visit course of treatment, a $20 fee may be charged for each subsequent cancellation or missed appointment.

  • If you miss more than three appointments without notice, you may be discharged from physical therapy.

  • We understand that emergencies can arise and will take extenuating circumstances into consideration.

  • Workers’ compensation insurers generally do not reimburse for cancelled or missed appointments. You may be personally responsible for these charges.

Prescription Policy: Most insurance companies require a valid prescription for physical therapy from a Connecticut-licensed physician, dentist, podiatrist, or advanced practice registered nurse. It is the patient’s responsibility to ensure that their prescription is current and valid.

Fees & Payments: It is our policy to collect copayments at the time of service. Coinsurance amounts are estimates because the final patient responsibility may not be known until your insurance claim has been processed. Estimates are based on typical patient responsibility.

If a balance remains after your insurance claim is processed, we will bill you for the difference between the amount paid and the patient-responsibility amount determined by your insurance plan. If you have an unmet deductible, payment may also be requested at the time of service.

Payment arrangements are expected at the time services are rendered unless prior arrangements have been made. We accept cash, checks, and all major credit cards. If we participate with your insurance plan, we will submit claims directly; however, payment remains the patient’s responsibility. We will assist you whenever possible with claim-related questions.

Medicare: Medicare no longer has an annual therapy-services cap. However, Medicare applies an annual documentation threshold. In 2026, when combined physical therapy and speech-language pathology services exceed $2,480, Medicare requires additional documentation confirming that continued services are medically necessary. Services may also be subject to targeted medical review after higher annual spending levels are reached. Patients remain responsible for applicable deductibles, coinsurance, and any non-covered services
